Les inhibiteurs de la transcriptase inverse, la didanosine (ddI), zalcitabine (ddC), la stavudine (d4T) et la lamivudine (3TC), peut provoquer une pancréatite. L'incidence est généralement faible, mais est d'environ 7% avec ddI, et jusqu'à 15% chez les patients pédiatriques donné le 3TC. Les Patients présentant des antécédents ou des facteurs de risque connus pour la pancréatite, tels que l'abus d'alcool ou d'hypertriglycéridémie, doivent être étroitement surveillés pendant le traitement par ces agents. Le traitement doit être interrompu dès les premiers signes ou symptômes évocateurs d'une pancréatite (p. ex., nausées, vomissements, douleurs abdominales, hyperamylasemia avec la dysglycémie, la hausse des triglycérides, diminution du taux sérique de calcium), et de préférence définitivement abandonnées si pancréatite clinique se développe.
