L'utilisation des inhibiteurs de l'HMG-CoA réductase est contre-indiqué dans les patients avec la maladie de foie active ou inexpliqués, élévation persistante des taux sériques de transaminases. Les inhibiteurs de l'HMG-CoA réductase sont largement métabolisé par le foie. Diminution du métabolisme des médicaments peut conduire à une accumulation et à un risque accru de toxicité, y compris des anomalies biochimiques de la fonction hépatique et, rarement, la jaunisse, l'hépatite, la cirrhose, les changements dans le foie, et une nécrose hépatique fulminante. Le traitement avec les inhibiteurs de l'HMG-CoA réductase doit être administré avec prudence chez les patients présentant des antécédents de maladie du foie et/ou une forte consommation d'alcool. Une plus faible dose initiale peut être approprié, et la surveillance clinique des transaminases hépatiques selon l'emballage du produit est recommandé. Les Patients qui développent une élévation des taux d'alat ou d'ASAT au cours du traitement doit être surveillé jusqu'à des anomalies de la résoudre. Si une augmentation de plus de 3 fois la limite supérieure de la normale persiste, la considération devrait être donnée à une réduction de posologie ou de retrait de la thérapie.
