Le mycophénolate subit complète métabolisme présystémique à sa forme active, la MPA . Moins de 1% de la MPA est excrété dans l'urine. Dans une étude à dose unique, l'aire sous la courbe (ASC) des amp dans les patients avec insuffisance rénale chronique sévère (clairance de la créatinine <25 mL/min) était d'environ 75% plus élevé que ceux des volontaires sains (clairance de la créatinine >80 mL/min). L'ASC du métabolite inactif a été de 3 à 6 fois plus élevé. Plusieurs études sur des doses n'ont pas été effectuées. Pas de données disponibles pour la transplantation cardiaque les patients atteints d'insuffisance rénale sévère et lorsque les avantages l'emportent sur les risques, le traitement par mycophénolate doit être administré avec une extrême prudence.
