Narcotiques (opioïdes) agents analgésiques sont largement métabolisé par le foie, et plusieurs d'entre eux (par exemple, la codéine, l'hydrocodone, la mépéridine, la méthadone, la morphine, le propoxyphène) ont métabolites actifs qui sont ensuite transformés à des substances inactives. Les phénothiazines sont largement métabolisé par le foie et peuvent s'accumuler dans les patients avec l'insuffisance hépatique. En outre, l'utilisation de certains phénothiazines a été associée à des effets hépatiques, y compris ictère cholestatique et élévation des enzymes hépatiques, généralement dans les premiers mois de traitement. Ictère survient généralement entre la deuxième et quatrième semaines de traitement chez environ 0,1% à 4% de tous les patients. La thérapie avec les phénothiazines doit être administré avec prudence chez les patients souffrant déjà de maladies du foie, des anomalies des enzymes hépatiques, ou de l'hépatite. La fonction du foie et de la bilirubine dans l'urine des tests doivent être effectués périodiquement au cours du traitement prolongé, et il faut aviser les patients de signaler immédiatement tout signe ou symptôme évocateur d'un ictère cholestatique du haut des douleurs abdominales, des nausées, peau jaune, grippe-comme des symptômes, des éruptions cutanées et de la fièvre. Phénothiazine, le traitement doit être interrompu, de préférence de façon permanente, si la jaunisse se produit et est imputable à la drogue. La guérison clinique est habituellement observé dans les quelques semaines suivant l'arrêt du traitement, bien que les modifications histopathologiques peuvent persister pendant de longues périodes.
