Les phénothiazines peut rarement causer une toxicité hématologique, y compris des cas d'agranulocytose, thrombocytopénie, éosinophilie, anémie aplasique, purpura, granulocytopénie, anémie hémolytique. Doux leucopénie peut se produire fréquemment avec de grandes doses pendant de longues périodes, mais est généralement réversible en dépit de la poursuite du traitement. La thérapie avec les phénothiazines doit être administré avec prudence chez les patients souffrant déjà de dyscrasie sanguine ou de la suppression de la moelle osseuse. La formule sanguine doit être obtenues régulièrement, et il faut aviser les patients de signaler immédiatement tout signe ou symptôme évocateur de dyscrasie sanguine tels que fièvre, maux de gorge, infection locale, saignement, pâleur, étourdissements, ou une jaunisse. La plupart des cas d'agranulocytose ont eu lieu entre le quatrième et le dixième semaines de traitement.
