Les diurétiques épargnant le Potassium peut provoquer une hyperkaliémie, ce qui peut entraîner des arythmies cardiaques mortelles. Les Patients atteints de diabète sucré, avec ou sans néphropathie, peut être particulièrement sensible à la hyperkaliémique effet de ces médicaments en raison d'un défaut dans le système rénine-angiotensine-aldostérone. La thérapie avec les diurétiques épargnant le potassium doit être évitée, si possible, dans les patients avec le diabète, en particulier incontrôlée, ou diabète sucré insulino-dépendant. Si ces médicaments sont utilisés, les taux sériques de potassium et de la fonction rénale doit être contrôlée à intervalles réguliers. La détermination des concentrations sériques d'électrolytes est particulièrement important lors de l'initiation du traitement, après un ajustement de la posologie, et au cours d'une maladie qui pourrait altérer la fonction rénale.
