Récepteurs de l'angiotensine II (AR) antagonistes peuvent provoquer une insuffisance rénale chez les patients dont la fonction rénale dépend de l'activité du système rénine-angiotensine-aldostérone système. En outre, une hypotension symptomatique peut survenir chez les personnes sensibles, qui peuvent compromettre rénale et myocardique de perfusion. Dans les cas graves, l'insuffisance cardiaque congestive (CHF), le traitement par l'AR antagonistes a été associé à une oligurie et/ou une azotémie progressive et, rarement, une insuffisance rénale, d'ischémie myocardique, et de la mort. La thérapie avec AR antagonistes doit être instauré avec prudence chez les patients atteints de graves CHF, en particulier lorsqu'ils sont accompagnés par le volume et/ou une déplétion sodique. Chez les patients qui subissent un déclin de la fonction rénale, l'arrêt de l'AR de traitement antagoniste n'est généralement pas nécessaire à condition qu'il existe une amélioration des symptômes de l'insuffisance cardiaque et rénale détérioration est bien toléré. Une hypotension transitoire n'est pas une contre-indication à la poursuite du traitement par l'AR antagonistes, puisque le traitement peut généralement être rétablie sans difficulté après la pression artérielle se stabilise.
