Récepteurs de l'angiotensine II (AR) antagonistes peuvent entraîner une hypotension symptomatique chez les patients avec une activation du système rénine-angiotensine, comme le volume et/ou de sodium-épuisement des patients. La thérapie avec AR antagonistes doit être administré avec prudence chez ces patients et à ceux qui sont prédisposés à hypovolémique ou victime d'hyponatrémie des états (p. ex., les patients sous traitement diurétique, surtout si des doses élevées ont été utilisés ou si récemment mis en place; ceux sur un régime hyposodé; patients sous dialyse rénale). Le Volume et/ou une déplétion sodique doit être corrigée avant de débuter un traitement avec AR antagonistes, et le patient doit être stable hémodynamiquement. Idéalement, les patients à risque d'hypotension excessive devrait lancer AR antagoniste thérapie sous surveillance étroite d'un médecin, de préférence avec une dose plus faible, et a suivi de près pendant les 2 premières semaines de traitement et chaque fois que le dosage de l'AR antagoniste ou de diurétique est augmenté.
