En général, l'utilisation d'anticoagulants par voie orale est contre-indiqué dans les patients avec un saignement actif ou d'une diathèse hémorragique ou d'autres risques significatifs pour les saignements, y compris de l'hémostase et/ou des troubles de la coagulation associé à l'hémophilie, hypoprothrombinémie, thrombocytopénie, thrombocytopathy, insuffisance hépatique sévère, et de troubles myéloprolifératifs comme la leucémie ou maladie de vaquez. En outre, les anticoagulants oraux sont généralement contre-indiqué en présence d'actifs à une ulcération de la gastro-intestinaux, respiratoires, ou des tracts génito-urinaire; une hémorragie cérébrale; anévrismes (cérébrale, dissection aortique); la péricardite et épanchement péricardique; endocardite bactérienne; et de l'éclampsie, pré-éclampsie, ou de menace d'avortement. Ces patients peuvent être à un risque accru d'hémorragie incontrôlable ou de complications hémorragiques au cours du traitement avec des anticoagulants oraux. D'autres possibles contre-indications incluent la diverticulite, la vascularite, la malnutrition, et de la vitamine C ou la vitamine K carences. La décision d'administrer des anticoagulants doit être basée sur le jugement clinique dans lequel les risques sont pesés contre les avantages de chaque patient.
