L'hépatotoxicité, y compris en cas d'acidose lactique, d'hépatomégalie grave avec stéatose, hépatite fulminante, et l'insuffisance hépatique, a été associée à l'utilisation de certains inhibiteurs de la transcriptase inverse (Inti) seul ou en combinaison avec d'autres agents antirétroviraux. La thérapie avec les Inti doivent être administrés avec prudence chez les patients souffrant déjà de maladies du foie, des antécédents d'abus d'alcool, ou de l'hépatite. La thérapie devrait être suspendue si la clinique ou de laboratoire des résultats évocateurs d'une acidose lactique ou prononcé une hépatotoxicité. L'utilisation de l'abacavir est contre-indiqué dans les patients avec insuffisance hépatique modérée à sévère que son innocuité et son efficacité n'a pas été établie chez ces patients.
