L'utilisation des oestrogènes est considéré par les fabricants et certaines autorités à être contre-indiqué chez les patients présentant une thrombose ou de troubles thromboemboliques. L'utilisation des œstrogènes de contraceptifs oraux contenant est en outre réputé contre-indiqué chez les patients présentant des antécédents de troubles et/ou de courant vasculaire cérébral ou une maladie coronarienne. Hypercoagulabilité et les changements dans les divers facteurs de coagulation et des composants sanguins ont été observés chez les femmes recevant la thérapie d'oestrogène. Bien que la signification clinique de ces effets est inconnue, les données épidémiologiques suggèrent qu'il peut être dose-dépendante. Le risque est probablement légèrement avec l'utilisation de nouveaux contraceptifs oraux faiblement dosés en l'absence de facteurs de risque connus (p. ex., tabagisme, en particulier au cours de l'âge de 35 ans, l'hypertension, l'hyperlipidémie, l'obésité, le diabète, l'âge de plus de 40 ans). Cependant, beaucoup plus de risque significatif n'a été rapporté avec des dosages plus élevés, tels que ceux utilisés pour traiter la prostate ou le cancer du sein métastatique ou ceux utilisé dans les formulations de pilules de contrôle des naissances. La thérapie avec ce traitement doit être administré avec prudence à la plus faible dose efficace et qu'après un examen attentif des risques et des avantages. Les estrogènes doit être évitée chez les patients présentant des antécédents de thrombose et de troubles thromboemboliques associés à l'utilisation d'œstrogènes, sauf lorsqu'il est utilisé dans le traitement du cancer du sein ou de la prostate tumeur maligne.
