Les benzodiazépines sont métabolisés par le foie, et les métabolites sont excrétés dans l'urine. Le chlordiazépoxide, clorazépate, diazépam, flurazépam et quazepam subir oxydatif N-désalkylation métabolites actifs qui sont beaucoup plus action que le composé parent. Ces métabolites sont ensuite soumis à de plus biotransformation pharmacologiquement inactifs produits avant l'excrétion par les reins. La thérapie avec des benzodiazépines doit être administré avec prudence à des doses initiales réduites chez les patients atteints d'insuffisance rénale et/ou hépatique. Les Agents qui sont convertis à faiblement active, à courte durée d'action, ou de métabolites inactifs peut être préférable dans l'insuffisance hépatique. Le lorazépam, oxazépam et le témazépam sont conjugués en métabolites inactifs, tandis que l'alprazolam, estazolam et le triazolam subir d'hydroxylation faiblement active ou des métabolites inactifs.

Tricycliques et tétracycliques antidépresseurs Tricycliques) sont connus pour subir le métabolisme dans le foie. Certains des métabolites, tels que ceux de l'imipramine, la clomipramine et la désipramine, peut être actif sur le plan pharmacologique. De nombreux métabolites sont également excrétés par les reins. Il y a très peu de données concernant l'utilisation du TCAs dans les patients avec insuffisance rénale et/ou une maladie du foie. La thérapie avec les Atc doit être administré avec prudence chez les patients atteints de manière significative altération de la fonction rénale ou hépatique. L'ajustement de la posologie peut être nécessaire.
