Anticholinergiques sont contre-indiqués chez les patients atteints de glaucome primaire, une tendance à l'glaucome (narrow angle de la chambre antérieure), ou des adhérences (synéchies) entre l'iris et le cristallin, ainsi que pour les personnes âgées et d'autres personnes en qui diagnostiqué un glaucome ou une pression excessive dans l'œil peuvent être présents. Parce que les anticholinergiques provoquer une mydriase, ils peuvent aggraver ces conditions.

Des agents sympathomimétiques peuvent induire transitoire mydriase via la stimulation de l'alpha-1 adrénergiques. Chez les patients avec anatomiquement des angles étroits ou glaucome à angle étroit, dilatation pupillaire peut provoquer une crise aiguë. Chez les patients atteints d'autres formes de glaucome, une mydriase peut parfois augmenter la pression intra-oculaire. Le traitement avec des agents sympathomimétiques doit être administré avec prudence chez les patients prédisposés à un glaucome, en particulier glaucome à angle fermé.
