L'utilisation de la cyclosporine est contre-indiqué chez les patients atteints de polyarthrite rhumatoïde ou le psoriasis avec des anomalies de la fonction rénale. La Cyclosporine causes réversible réduction du débit sanguin rénal et de la filtration glomérulaire, résultant en une augmentation de la créatinine sérique et de l'azote uréique du sang. Doux néphrotoxicité généralement répond aux réductions de doses de cyclosporine. Persistant, chronique, progressive et la néphrotoxicité a eu lieu. Biopsies rénales à partir de ces patients peut démontrer la fibrose interstitielle, atrophie tubulaire, globale ou segmentaire de la glomérulosclérose, ou lisse vasculaire des lésions musculaires. De plus grandes doses cumulatives ou d'élévation de la cyclosporine concentrations minimales peuvent être associés avec le développement de la fibrose interstitielle. La surveillance clinique de la fonction rénale est nécessaire et la différenciation entre la cyclosporine néphrotoxicité induite, le rejet d'allogreffe, et d'autres causes d'altération de la fonction rénale doit être déterminé avant le début de la cyclosporine ajustements de la posologie.
