Des récepteurs bêta-adrénergiques bloquants (aka les bêta-bloquants) en général ne devrait pas être utilisé chez les patients ayant manifeste une insuffisance cardiaque congestive (CHF). La stimulation sympathique est peut-être important dans le maintien de la fonction hémodynamique chez ces patients, ainsi bêta-adrénergiques peuvent aggraver l'insuffisance cardiaque. Cependant, le traitement par bêta-bloquants peut être bénéfique et peut être administré avec prudence chez certains CHF patients à condition qu'ils soient bien rémunérés et recevant des digitaliques, diurétiques, un inhibiteur de l'ECA, et/ou des nitrates. Le carvédilol, en particulier, est indiqué pour une utilisation avec ces agents dans le traitement de l'insuffisance cardiaque légère à sévère de la ischémique ou cardiomyopathiques origine. Il est également de plus en plus de preuves que l'ajout d'un bêta-bloquant à la norme thérapie peut améliorer la morbidité et de la mortalité chez les patients avec insuffisance cardiaque avancée, même s'il est incertain de savoir si l'efficacité varie de façon significative avec les différents agents.
